The following information is needed for the Return To Duty Drug Screen:
- Your name
- Your Email
- Phone Number
- A Copy of Your Driver License
Once I have all the required information, your employer sponsor will order the QPASS which comes through email so make sure you provide a good email address!!
$185.00 includes the Cost of your return to duty test ** your SAP may order drugs, alcohol or both in the event of alcohol testing add an additional 35.00
